Check whether TapNoise can play a preview cue before investigating the agent. That separates an audio problem from a hook or session problem.
Test the app’s playback
Open Settings › Agents and preview Done. Inspect Play agent sounds, output volume and the selected audio device. If the microphone is active, Quiet during calls may intentionally silence playback.
A silent preview is not evidence that Claude Code, Codex or Cursor failed to run its hook. Fix the playback path first.
Inspect the specific connection
Look at the status beside the agent you are using. If TapNoise reports an outdated or moved hook, choose Repair. Keep the app in Applications rather than connecting from the mounted download image.
Start a new local agent session after changing the configuration. Existing processes can retain earlier settings, so test the new connection with a small turn rather than reusing a long-running session as your only evidence.
Test a supported event
Use a completed turn for the first test. Do not test Codex or Cursor by waiting for Claude Code’s separate permission cue; TapNoise’s event coverage differs between agents.
If playback works and the connection looks healthy, inspect the agent’s own hook diagnostics and configuration. Preserve unrelated entries. For a moved app, the repair guide is more targeted than deleting an entire settings file.
